Please complete a Travel Risk Assessment Form at the bottom of this page. This form needs to be received by the surgery AT LEAST 8 weeks before you travel.

Our nurses will need to review your medical record and then they can provide travel health advice and arrange for appropriate immunisations.  Some vaccines need time to take effect and some may require a course over several weeks.
